Parapelvic cysts are uncommon and found in ∼1.25–1.5% of autopsy cases . Parapelvic cysts are usually solitary, originating from renal parenchyma and protruding into the sinus. Because of the hypoechoic character by ultrasonography and the special location in the renal sinus, parapelvic cysts are often misdiagnosed as hydronephrosis .

Renal sinus cysts (sometimes referred to as peripelvic cysts) are asymptomatic cysts that arise from the lymphatics of the renal hilum. Peripelvic renal cysts defined as cysts which are situated in the renal hilus, intimately related with the renal pelvis and calyces. It can be simple or multiloculated and single or multiple. Parapelvic cysts are rare and often benign and must be differentiated from hydronephrosis. The possibility of a parapelvic cyst must be kept in mind when the asymptomatic hydronephrosis is noted by ultrasonography with unknown causative obstructive lesion. Further image study such as CT scan is needed for the correct diagnosis.
They are usually single or few and resemble simple renal cortical cyst in morphology. A parapelvic cyst at times may cause compression of the pelvicalyceal system resulting in hydronephrosis. Excerpt Peripelvic cysts of the kidney may be defined as cysts which are situated in the hilus of the kidney, intimately associated with the renal pelvis and calyces. In a patient who had undergone emergency nephrectomy during exploratory surgery, a pathologic diagnosis of peripelvic lymphangiectasia was made. Subsequent computed tomographic examination of the remaining kidney showed large multilocular renal sinus cysts, renal capsular cysts, and regions of fluid attenuation surrounding the mesenteric vessels, findings consistent with peripelvic lymphangiectasia. Peripelvic cysts which are large enough to cause symptoms or be detectable on radiographs are rare and have been seldom reported in the urological or radiological literature. Smaller multiloculated hilar cysts, however, which are not detectable in vivo are not unusual, being said to occur in over 1 per cent of routine autopsies (1). Henthorne (3) found 20 cases of peripelvic cysts at autopsy. Renal sinus cysts are benign.

If one finds echolucent structures in the renal sinus, which points argue for renal sinus cysts?

While usually an incidental radiographic finding, these cysts may obstruct the collecting system and yet not be apparent on n Cysts are probably the most frequent abnormal findings to be described in kidney imaging reports. If they are few and small in an elderly patient, they may be ignored as a natural aging process because incidence of simple cysts increase with age, reaching above 50 % after the fifth decade. Peripelvic cysts are thought to arise by lymphatic ectasia and are usually multiple and septated. Peripelvic lymphangiectasia is rare and is associated with perirenal lymphatic cysts, as well as perivascular cysts (probably dilated lymph vessels). 23,24 The perirenal cysts appear to arise in a capsular location rather than a cortical location. Perirenal serous cysts of müllerian origin are uncommon lesions that may present as large symptomatic perirenal cystic masses mimicking exophytic renal cysts on CT. Serous cyst of müllerian origin should be considered in the differential diagnosis when CT reveals a large solitary perirenal cyst in a woman with flank or abdominal pain or both.
Parapelvic Cyst Symptoms There are no serious and critical parapelvic cyst symptoms on which the treatment is started for complete recovery. However, there are some critical health conditions of patients which help the doctors to identify the disease as a parapelvic cyst.
Unlike the common simple cysts of the kidney parenchyma, peripelvic cysts are not buried within the renal parenchyma nor are they present as a mass in either pole or along the external surface of the kidney… Peripelvic cysts are a type of renal sinus cyst. From the case: Peripelvic cysts.
